The goal of this questionnaire is to obtain information to ensure compatibility between your family and the pet you'd like to adopt. By answering these questions thoroughly and honestly we will be able to determine if the animal you are interested in would be a good fit for your lifestyle.  We simply want to make sure you get the right pet for you/your family.  You must be 21 years of age or older to adopt one of our animals.
